The Department for Business, Innovation and Skills (BIS) have updated the paper version of the form to be used by individuals.

This should be used if they have concerns about:

an employment agency working time limits (working more than 48 hours a week) the national minimum wage the minimum wage when working in farming or agriculture

The form can be accessed here.

Those completing the form do not have to give their name or any other personal details.

Once completed, the form should be emailed to fpt.hmrc@hmrc.gsi.gov.uk

Please note: - the online form has been temporarily withdrawn.

These forms are to be used for reporting allegations or suspicions of National Minimum Wage or Working Time Regulation violations.

There are several ways of registering a complaint:

By telephone to acas 0300 123 1100 - Monday to Friday, 8am to 8pm and Saturday, 9am to 1pm, or By making a direct complaint about the employer to the relevant government enforcement body as follows: HM Revenue and Customs (HMRC) - about the National Minimum Wage Employment Agency Standards inspectorate - about employment agency legislation (except Agency Worker Regulations) Gangmasters Licensing Authority (GLA) – about agency workers in agriculture, horticulture, shellfish gathering or associated processing and packaging Health and Safety Executive (HSE) - about working time issues, including maximum weekly working hours the Agricultural Wages Team in the Rural Payments Agency - about agricultural wages
